Dear Parent and/or Guardian,
Health and safety will continue to be our main priority as we welcome students back to Bancroft this coming school year. In order to better inform our new health and safety protocol planning, it is important that we have the most up to date student health information.
As part of your preparation for the upcoming school year for your student, please note the following information regarding required health documentation. All health information is due to the school nurse by August 16th, 2021.
Health Center Forms
- Health Physical Examination — Students entering PreK, Kindergarten, Grades 4, 7, and 9, new students, all international students, and all students participating in athletics need a current, up to date physical exam form from their physician. Physicals should be submitted via email to the school nurse Liza Marzilli, BSN, RN. Physical examination reports expire twelve months from the date of the exam and must be current.
- Required Immunizations — Please reference this updated list of required immunizations from the Massachusetts Department of Public Health. Bancroft School requires all students to be fully immunized per the Massachusetts Department of Public Health regulations (CMR 220.000) unless the student qualifies for a medical or religious exemption as stated in these regulations.
- International Students — In addition to a current physical exam and immunization record, all international students need documentation of a negative Mantoux tuberculosis test (PPD) or QuantiFERON Gold TB test (preferred) done within the past three months prior to the start of classes. Any follow up testing, such as chest x-rays, need to be completed prior to being on campus. All documentation needs to be in English.
- COVID-19 Vaccination — We strongly recommend that eligible students receive any of the approved COVID-19 vaccinations available. If students have already received their vaccination, please provide a copy (photo or scan) of their vaccination card to School Nurse Liza Marzilli, BSN, RN (if not already included on the immunization record).
- Influenza Vaccination — Annual vaccination is strongly encouraged for each flu season. Please send documentation to school nurse Liza Marzilli, BSN, RN once completed.
- Vision Screening Guidelines (Kindergarten students) — Upon entering or within 30 days of the start of the school year, Kindergarten students must have documentation of having passed a vision screening test within the previous 12 months. This should come from their physician.
Medications
Bancroft School follows Massachusetts State law requiring appropriate management of medication in the school setting. If your child requires any over the counter, prescription, or rescue medication while at school, BOTH of the following forms must be completed annually. A responsible adult should deliver medications in a pharmacy or manufacturer labeled container to the school nurse.

Parents of students with any other medical conditions or concerns such as celiac, diabetes, seizures, etc. should contact the school nurse, Liza Marzilli, BSN, RN, prior to the start of school to develop the appropriate action plan. All health information in the Bancroft Health Center is confidential. It is shared on a need-to-know basis with permission from the parent or guardian, except in the case of an emergency or life-threatening condition.
